Abstract Book
Abstract book (only in Japanese) is available at a price of JPY 2,000 during the Conference.
The abstracts will be available online on the congress website in May 2016.
Lunch Box
Lunch box can be purchased through the online registration.
You will receive it at Tokyo Metropolitan Indusrial Trade Center Taito-kan.
Lunch box can be purchased at Asakusa View Hotel on the day, first come, first served. Menu of lunch box may differ by the venues.

Please read the below instructions to register on-line for the 7th Annual Conference of the Japan Primary Care Association.
Pre-registration
Early registration is available only on-line. Please click Registration at the bottom of the page to gain access to the application form. Please register only once. Those who have completed their registration will receive an email. This email is necessary for exchanging it for the registration badge. Please print out the email and bring it with you on-site.
Payment
Payment can be made only by credit card.

Regular Registration： noon JST, Thursday, May 5, 2016 noon JST, Friday, May 6, 2016 - *Registration will be completed when the payment is confirmed.
If there is no payment confirmed even after the deadline, on-line registration will automatically be cancelled.
Receipt
Registration badges which are combined with receipts will be distributed on-site.
